Our mission at SVDP Milwaukee is to eliminate poverty and hunger throughout Milwaukee County. We're here to help you - whether it be directing you to one of our two Meal Program locations or referring you to one of our neighborhood SVDP volunteer groups .
We are here to help you! To request assistance with basic needs such as food, clothing,
and household necessities, please call 414-462-7837 and leave a message on extension 110.
Our intake line is monitored Monday through Friday between 9am-3pm.
Due to the large volume of calls we receive; we may not return your call for up to 5 days.
If we are able to make a referral to one of our volunteer groups, it may be up to 6 weeks
before they can respond with a home visit due to the many requests that they receive.
To place a request for assistance, please state your name, address, phone number,
and what you need assistance with, and we will return your call as soon as possible.

- Complete this form (below) online and submit to our Intake Department
**** PLEASE NOTE, IF THIS FORM IS NOT COMPLETE, IT MAY DELAY POSSIBLE SERVICES*** - ALSO, THE ADDRESS PROVIDED ON THIS FORM HAS TO BE THE ADDRESS WHERE YOU NEED THE HELP. IF YOU ARE MOVING SOON, YOU NEED TO WAIT UNTIL YOU ARE MOVED AND THEN SUBMIT YOUR REQUEST.
- Intake staff will either return your e-mail or call and discuss your request if further information is needed.
- If a referral is able to be made, a SVDP volunteer will call you within 1 - 3 weeks to arrange a home or phone visit and meet with you about the item(s) requested.
- If you are unable to complete this form, you may call 414 462-7837, extension 110. You do not need to call if you have completed this form.
All information provided is held in strict confidence and is never shared with any other agency.
